Whether your family is familiar with shift care services or you are new to receiving shift care services, we partner with your Primary Care Physician and Specialist(s) to provide tailored care specific to your needs in the comfort of your home.
Our broad range of highly competent clinicians can provide care and treatment for sub-acute illnesses that include but are not limited to Tracheostomy, continuous nutritional feeds through a G-tube/J-tube, oxygen dependence utilizing Mechanical Ventilation machines, TPN, complex medical needs such as seizures and congenital disorders.
Your care is provided by a licensed Vocational Nurse or Registered nurse. This is shift care based with a set schedule throughout the day and in some cases at night.
What Can I Expect During The On-Boarding Process in 3 Simple Steps

A nurse supervisor will contact you to perform an initial assessment and fill out necessary consent forms and paperwork. We will gather all necessary medical history and current medications to build a care plan to review with your physician and specialist.

A care coordinator will contact you to determine the required schedule and begin the process to match you with a nurse/team of nurses.

Once a nurse is matched, we conduct a start of care and orientation with the nurse and family to review Plan of Care details required for the nurse to perform. A Case Manager will oversee this care process along with a monthly Supervisory visit from a Registered Nurse (RN)
