You may qualify for Home Health skilled nursing visits if you
- Have difficulty or it is a taxing effort to leave the home
- Are Homebound due to illness, surgery, or injury
- Have recently been discharged from the hospital
- Are making frequent trips to the physician’s office
- Are under the care of a physician who has determined that in-home care is medically necessary
- Need periodic bloodwork
- Have been perscribed a new medication
- Have a blood pressure that fluctuates
- Are an unstable diabetic
- Have wounds that are not healing or frequently infected
- Are coming from a nursing home (Medicare will pay for continued therapy in your home)

What Can I Expect During The On-Boarding Process in 3 Simple Steps

A nurse supervisor will contact you to perform an initial assessment and fill out necessary consent forms and paperwork. We will gather all necessary medical history and current medications to build a care plan to review with your physician and specialist.

A care coordinator will contact you to determine the required schedule and begin the process to match you with a nurse/team of nurses.

Once a nurse is matched, we conduct a start of care and orientation with the nurse and family to review Plan of Care details required for the nurse to perform. A Case Manager will oversee this care process along with a monthly Supervisory visit from a Registered Nurse (RN)
